Do I need an appointment?

You must apply in person using Form DS-11 if at least one of the following is true:

  • You are applying for your first U.S. passport
  • You are under age 16
  • Your previous U.S. passport was issued when you were under age 16
  • Your previous U.S. passport was lost, stolen, or damaged
  • Your previous U.S. passport was issued more than 15 years ago

If none of the above statements apply, you have to Renew by Mail using Form DS-82.

Passport Fee Information:

Passport fee is payable to: U.S. DEPARTMENT OF STATE (check or money order only).

Processing fee is payable to: City of Sammamish (cash, check, Visa and Mastercard).

Note: All applicants must appear in person. Notarized or unofficial photocopies of official documents (e.g. birth certificate, marriage certificate, adoption decree) are not acceptable. The documents submitted must be the original or certified copies from the issuing agency.

In addition, children under 16:

  • Must present evidence of parental relationship (birth certificate, adoption decree) in addition to citizenship evidence (passport, birth certificate).
  • Are affected by the Two-Parent Consent Law.
